    Stopping plaquenil for surgery

In order to minimize the risks of infection and other potential complications with podiatric surgery in this patient population, this author reviews the literature and offers insights on whether medications for RA should be withdrawn, continued or modified during the perioperative period. Increasingly, the treatment of rheumatoid arthritis includes the utilization of immunosuppressive therapeutic agents.

That said, there's no reason for patients to stop using Plaquenil just because they will have cataract surgery, Dr. Osborn said. He sometimes sees SS patients who have read about Plaquenil's side effects and ask if they have to stop the drug prior to cataract surgery. However, he knows of no specific contraindications with cataract surgery. You have been scheduled for an epidural steroid injection which is useful to treat pain and determine if your pain is coming from the spinal nerves. You will be receiving an injection of local anesthetic, corticosteroids, and a small amount of contrast dye. I just had major spine surgery in November. The surgeon had me check with my rheumy about stopping Plaquenil and I was told to continue it, or just stop it the day of surgery then get right back on, I asked what would happen if I stopped for a week and was told I'd risk going into a flare. I stayed on it and surgery went OK.

Louie and Ward demonstrated that in the state of California, rates of joint surgery for the treatment of RA declined in the 1990s and have continually diminished since that time.1 The authors concluded that the favorable long-term outcomes for the treatment of rheumatoid arthritis with newer agents have resulted in a decreased need for bone and joint surgery in patients afflicted with this disease. The effectiveness of disease modifying agents in reversing the progression of rheumatoid arthritis (RA) is illustrated by the reduced frequency with which patients with RA undergo surgery for bone and joint deformities.

Overall, individuals with rheumatoid arthritis have a higher risk of myocardial infarction which is similar to individuals with diabetes mellitus or a person 10 years older than the age of the patient.

    • What is the correct way to discontinue use of plaquenil?.

Recently there have been a few cases in which patients of mine have been referred for orthopaedic procedures in the context of active Rheumatoid arthritis RA. The orthopaedic surgeon has expressed a real concern about the use of Methotrexate MTX in the peri-operative period, resulting in the patient being very anxious to stop the medication. The issue raised is whether MTX must be discontinued temporarily in the perioperative period. It is my impression that most rheumatologists discontinue MTX during the perioperative period -- generally the week of surgery and for 1-2 weeks post surgery. My rheumy's assistant left me a message to tell me to stop 2 weeks before my procedure, but didn't explain why. one more question for those of you who had surgery aside from going off or staying on plaquenil, did the surgery cause any noticeable flare up of your symptoms? i will certainly update you once i am back home and online again.
