Is chloroquine safe in early pregnancy

While the mechanism is poorly understood, pregnant women have a reduced immune response and therefore less effectively clear malaria infections. In addition, malaria parasites sequester and replicate in the placenta.

With both drugs, there is increased risk for birth defects and miscarriages. Methotrexate should be discontinued at least three months prior to becoming pregnancy and leflunomide should be avoided for two years before pregnancy. 2. Anti-malarial drugs hydroxychloroquine, chloroquine are considered pregnancy The pharmacokinetics of most antimalarial drugs are also modified in pregnancy and dosages will need to be adapted. Other factors, including HIV status, drug interactions with antiretrovirals, the influence of haematinics and host genetic polymorphisms may influence safety and efficacy. You can take some anti-malaria medicines safely during pregnancy, but should avoid others. For example Chloroquine and proguanil usually combined can be used in pregnancy, but may not offer enough protection against malaria in many regions, including Africa. You'll also need to take a 5mg supplement of folic acid if you're taking proguanil.

Malaria infection during pregnancy can lead to miscarriage, premature delivery, low birth weight, congenital infection, and/or perinatal death. Pregnant women are three times more likely to develop severe disease than non-pregnant women acquiring infections from the same area.

  5. The purpose of this study is to test prevention strategies for pregnancy-related malaria. Researchers will compare different malaria treatments and treatment schedules which include chloroquine therapy weekly doses versus being dosed twice during pregnancy for 3 days each time to the standard practice of preventive treatment intervals in pregnancy with the drug sulfadoxine-pyrimethamine.

Chloroquine has not been formally assigned to a pregnancy category by the FDA. There are no controlled data in human pregnancies. Congenital anomalies were reported in the offspring of one woman being treated with chloroquine 250 to 500 mg daily during pregnancy for SLE; however, chloroquine has been used in the prophylaxis and treatment of malaria during pregnancy without evidence of fetal harm. For pregnant women diagnosed with uncomplicated malaria caused by chloroquine-resistant P. falciparum infection, women in the second and third trimesters can be treated with artemether-lumfantrine, and for all trimesters, mefloquine or a combination of quinine sulfate and clindamycin is recommended. Quinine treatment should continue for 7 days for infections acquired in Southeast Asia and for 3 days for infections acquired elsewhere; clindamycin treatment should continue for 7 days. Nov 25, 2019 Chloroquine is usually taken once per week on the same day each week. Continue taking the medicine regularly during your stay and for at least 8 weeks after you leave the area. If you stop using chloroquine early for any reason, talk to your doctor about other forms of malaria prevention.
